Your Support Plan and You.
We aim to deliver an individual and fair service to our tenants which enables living as independently and safely as possible within their own home. Often this is delivered by working with other services and agencies who will provide at home support such as cleaning, helping people bathe and dress, cooking meals etc. We also work alongside professional Health Services and advocate on your behalf to maximise your health and safety.
We will review your support plan with you on a six monthly basis and update and changes in needs or choice, we will however review this sooner if your health deteriorates or your needs have changed. If you feel that you do not need or wish to have a support plan completed at this time we request that you complete a Support Plan disclaimer form which enables us to monitor our performance. Should you change your mind on this please let us know and we will arrange to visit and get a support plan updated with you.
